Atlas of Emergency Radiology

Designed to be an atlas rather than a descriptive text of emergent radiology, this work for emergency physicians and ambulatory care givers focuses on technical aspects of radiographic imaging and helps practitioners through difficult cases that involve radiologic interpretation of plain films of multiple body systems. Illustrated with numerous b&w images, chapters cover cardiothoracic, abdominal, skull and facial, spine, pelvic and lower extremity, upper extremity, and pediatric radiography, and radiography of bone loss. Johnson teaches in the Department of Emergency Medicine at the State University of New York Upstate Medical University. This atlas contains radiographic images that are likely to be encountered in emergency medicine and ambulatory care. The images are grouped in anatomic categories and introduced with a minimal amount of text. The purpose is to help emergency physicians and ambulatory caregivers interpret plain films so they can make efficient clinical decisions. Since emergency care providers are often required to provide preliminary interpretations of radiographs these objectives are worthy. This book is essentially written by an emergency physician for emergency physicians. The main editor is certainly a credible authority on the needs of emergency physicians; however his expertise in emergency radiology is not documented. Two of the four editors are Associate Professors of Radiology. Their contributions to the text are not clear. As radiologists, they would presumably be knowledgeable about the subject matter, yet neither of them wrote any of the chapters. All the chapters but one were authored by Dr. Johnston. This book focuses on a variety of areas in emergency radiology, including radiography of the chest, abdomen, cranium, spine, and skeletal system. The photos, a strength of this book, are overall high in quality and extensive. There are weaknesses within the text, which is incomplete and inaccurate in places. For example, the discussion in the pulmonary disease/pulmonary embolus section deals almost entirely with pulmonary embolism; there is only brief mention of obstructive lung disease. Images and some description of emphysema, bronchitis, asthma, and atelectasis should have been included. An image of complete left lung atelectasis is provided but misplaced in the section of pneumonia.Inaccurate statements are perhaps of more concern. For instance, in the trauma section, a left apical cap is erroneously described as blood in the pleural space; an apical cap is actually extra pleural hematoma extending from the mediastinum, which is much more ominous. Elsewhere in the book, the author states that air bronchograms are unlikely to be seen in noncardiogenic pulmonary edema and this is a simply not true. Air bronchograms can be present whenever there is filling of the alveolar spaces around an air filled bronchus. Another problem is that the text refers at times to incorrect and misplaced figure numbers. While this book may be useful to emergency providers as an atlas, the text problems are concerning and raise questions as to the quality of the editing and the role of the radiologist editors. Emergency providers no doubt have a need for reference books such as this. Perhaps subsequent editions will demonstrate more input from radiologists and enhance the overall value of the book.
