This book provides advanced care providers in all settings with a practical and comprehensive textbook on site-specific cancer care. Also included are well-designed chapters on complications of cancer treatment, pain and symptom management, hospice care, and health promotion for cancer survivors. The text purports to "" ... facilitate the work of advanced practitioners and the care of their patients in all primary care settings from rural areas to highly specialized tertiary care centers."" It is meant to be a resource for advanced care providers (defined by the editors as physician assistants, nurse practitioners, and clinical nurse specialists). The editors hope to enhance the quality of care received by patients with cancer, and this goal is worthy. While other specific texts on cancer practice such as Rosenberg's Cancer: Principles & Practice of Oncology , 5th Edition (Lippincott Williams & Wilkins, 1997); Groenwald's Cancer Nursing: Principles and Practice, 4th Edition (Jones & Bartlett, 1997); Holland's, Cancer Medicine - 2 Volume Set, 4th Edition (Williams & Wilkins, 1997) are available to the advanced practitioner, it is a pleasure to have a concise, site-specific text which addresses the spectrum of cancer care which faces the advanced practitioner. This text is written for advanced practitioners. It is an appropriate text for physician assistants, nurse practitioners, clinical nurse specialists, and for clinical nurses who specialize in oncology/hematology. It would be a welcome addition to any inpatient unit or ambulatory setting where oncology/hematology practice is the primary focus. It wouldalso be a useful tool as a textbook in a subspecialized advanced practice training program. This book contains individual chapters on site-specific cancers that address epidemiology and risk factors, pathology and histology, clinical presentation and physical examination, diagnostic evaluation, treatment, and after-treatment surveillance. There are well-designed, well-placed tables and black-and-white illustrations and photographs. The index and references are excellent. The editors have taken a stance of primary care as their focus -- hence the title. However, oncology care frequently presents and is managed in the secondary and tertiary settings. Some chapters address the continuum of care better than others; some stop with the initial referral. This is a work that has significant value for advanced practitioners (both primary care and oncology-based) and their patients. Therefore, I would like to see subsequent editions expanded. Such an effort could arguably include in-depth discussions of the major cancer treatment modalities (surgery, radiation therapy, chemotherapy, progenitor blood stem cell transplantation, and biotherapy) either alone or in conjunction with the site-specific cancer for which they are the principal treatments. I would like to see a broader treatment of the psychological, social, educational aspects of cancer care, as well as professional issues, research, and resources for the advanced practitioner. In such a case, I would also like to see a more encompassing title and a hardcover edition. This is an expansive work with great merit. I have already used it extensively in my own practice and I look forward to the ongoing excellent work that comes from the dedicated professionals at the MD Anderson Cancer Center.
